The Dunn County Sheriff’s Office is hiring a full-time Project Hope Case Worker to join our dedicated team supporting individuals who are deflected or diverted from the justice system through the Project Hope Law Enforcement Deflection and Diversion Program.

In this role, you'll work side-by-side with our Behavioral Health Officers (BHOs) to deliver individualized case management services to participants and individuals reentering the community from jail. You’ll help build reentry plans, coordinate referrals, and provide ongoing support—bridging the gap between law enforcement and behavioral health services.

Note: This position is funded through a state Department of Health Services Law Enforcement Opioid Abatement grant that is renewable annually, currently funded through June 30, 2026, anticipating applying for extension.

This is an hourly, non-exempt case worker position in the Sheriff’s Office, with an anticipated starting wage is between $27.78 and $29.32 per hour.

The following duties are primarily performed and are essential for this position. Employees are expected to be able to perform each of these job duties satisfactorily and successfully to be qualified for the position. Other duties may be required and assigned.

  • Develop individualized case plans in collaboration with participants, focusing on goals such as sobriety and recovery, mental health stabilization, basic needs, and overall wellness
  • Assist clients in adhering to case plans, such as setting up appointments, arranging for transportation to appointments, and providing support
  • Provides pre-release reentry case management support and services to people leaving jail
  • Collaborate with other criminal justice professionals and programs to best serve the client
  • Provides assistance over the phone or in person to the program participants. The assistance may be answering questions about the community or county programs, assistance in accessing supports to meet basic needs, providing linkage to resources and services, facilitation, and support through emotional and traumatic situations.
  • Receives phone, in-person, or electronic reports, referrals, and requests for services and resources.
  • Makes referrals, maintains contact with other agencies/organizations, coordinates services with a variety of providers.
  • Completes accurate and timely record keeping including case notes, screeners and assessments, treatment plans, crisis plans, court recommendations, and activities.
  • Attends staff training and other meetings as required.

AREA SPECIFIC FUNCTIONS

  • Provide case management services to participants
  • Assist participants in applications for benefits and / or services
  • Assisting in preparing grant reports and data collection required by funders
  • Provide services in support of treatment and recovery
  • Provides follow up and linkage to services for participants who access services through Project Hope
  • Serves as liaison between service providers, law enforcement, and other community stakeholders
  • Develops case plans collaborative with participants
  • Monitor participant compliance and follow through
  • Educate clients and community members about substance use disorders and co-occurring mental health disorders and available community resources
  • Aid family members to assist them in understanding, dealing with, and supporting the client
  • Attend trainings, seminars, and conference related to professional development, industry trends, and industry standards and best practices. Occasional overnight travel is required.
  • Participate in public relations events to promote the mission of Project Hope and educate the public to reduce stigma surrounding substance use disorders and mental health conditions.
  • Collaborate with other Project Hope partners including Menomonie Police Department, county human services, peer support, treatment providers, and other support services and agencies.
  • Maintain accurate and confidential documentation of client interactions, progress, treatment/wellness plans, and outcomes.
EDUCATION AND/OR EXPERIENCE REQUIREMENTS
  • Bachelor’s degree in Criminal Justice, Rehabilitation, Social Work, Sociology, Psychology, Human Development, or similar field.
  • Minimum of one (1) year previous experience in Case Management or Human Services.

LICENSES, CERTIFICATIONS, AND OTHER REQUIREMENTS

  • Valid driver’s license or the ability to travel between work locations.
  • Prior to hire, a background check including criminal record history will be conducted. Information from the background check will not necessarily preclude employment but will be considered in determining the applicant’s suitability and competence to perform in the position.

PREFERRED QUALIFICATIONS:

  • Criminal justice system experience.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES QUALIFICATIONS

  • Thorough knowledge of the principles, techniques and trends in social services.
  • Thorough knowledge of local and state regulations, rules, policies and procedures; thorough knowledge of conducting assessments, making referrals and developing service plans.
  • Thorough knowledge of conducting and preparing court studies, court documentation and providing testimony.
  • General knowledge of standard office equipment; general knowledge of standard office software.
  • General knowledge of electronic case management and database systems.
  • Ability to apply professional judgement, discretion and decision making.
  • Ability to establish and maintain effective working relationships with supervisors, providers, program participants, outside agencies and other professionals.
  • Ability to communicate effectively both verbally and in writing with associates, supervisors, clients, court officials, attorneys, federal and state agencies, schools, law enforcement, medical providers, community service providers.
  • Ability to follow Dunn County policies and procedures including those pertaining to confidentiality.
  • Ability to prioritize and organize job assignments.
  • Ability to manage time effectively, manage multiple projects, and complete work within established deadlines.
  • Ability to work the allocated hours of the position.
  • Knowledge of the NASW Code of Ethics and MPSW 20 Code of Conduct and professional application of these principles and statute.
  • Case workers will be held to the same professional standard, statutes, Code of Conduct, and Code of Ethics as social workers, to include 4 hours of continuing education in Ethics and Boundaries bi-annually.
