The Re-entry Care Coordinator (rCC) is responsible for providing chemical dependency treatment services and case coordination to incarcerated adult and juvenile individuals. The rCC conducts assessments, facilitates recovery services, and coordinates care during and after incarceration to promote successful reintegration into the community. This position is a key link in ensuring continuity of care and compliance with state and federal treatment standards.

Typical Duties
  • Perform assessments on referred clients and determine their amenability for the program or referral to other services.
  • Develop detailed case reports for use by courts; may testify and be subject to cross-examination.
  • Provide chemical dependency treatment services in the Clallam County Jail and Juvenile Detention Center.
  • Integrate treatment model with Department of Behavioral Health and Rehabilitation’s requirements.
  • Ensure completion of Substance Use Disorder Pre-Screen and Opioid Use Questionnaire by all incarcerated individuals.
  • Complete assessments and coordinate release services in the community.
  • Collaborate with care coordinators, probation officers, and providers to support recovery-oriented care.
  • Refer and coordinate transitions to in-patient treatment as needed, including post-return treatment continuation.
  • Deliver relapse prevention training and support relapse recovery interventions.
  • Maintain detailed client files and electronic health records (EHR); ensure compliance with federal and state documentation standards including 42 CFR.
  • Prepare and submit accurate documentation for billing and reporting purposes.
  • Attend and participate in team meetings, wrap-around services, and case reviews.
  • Meet annual training and continuing education requirements to maintain certification.
  • Ensure compliance with applicable laws, regulations, and internal policies.
  • Perform other program-related duties as assigned by the Re-entry Targeted Care Manager (rTCM).

Education and Experience

  • Must be 21 years of age or older.
  • Associate’s degree in chemical dependency, social services, or closely related field is required; Bachelor's degree preferred.
  • One year of experience in chemical dependency counseling and assessment preferred.
  • Experience working with teens and families preferred.
  • Washington State Certification as a Substance Use Disorder Professional (SUDP) is required.
  • Ongoing education to maintain SUDP certification is required.
